Why does the saved picture have no file extension

This may depend on the operating system you're using. For example, on Linux if a sandboxed environment (snap or flatpak package) is being used, Pinta cannot add the extension without running into serious problems (e.g. bug #1958670) so it's up to what your system's file browser behaviour is.
