[Lenovo ThinkPad T520] Booting Trouble

Bug Description

I have been having booting trouble with my Lenovo for months. I have consistently left 12.04 and had to use other systems. Sometimes I am forced to return and try to see if this issue has been fixed, it has not. I have not been able to find any support for my issue, this is my last hope.

Symptoms:
  1) Sometimes after grub, boots to purple screen and stays there
  2) Sometimes after grub, and purple screen, starts booting... freezes after (firewire loaded device) (however, I believe this is just the last "status" message
  3) the newest problem, after the firewire line, starts complaining with the following lines:

[ xx.xxxxxx] ata1.00: revalidation failed (errno=-5)
[ xx.xxxxxx] ata2.00: revalidation failed (errno=-5)

 4) Sometimes I boot successfully
 5) Sometimes I try to boot through recovery mode and sometimes it works and sometimes it doesn't.

Notice the amount of "sometimes", this is because this bug is incredibly random yet I USUALLY do not boot. Please help me out, I'll be glad to get whatever info you need, but I have no clue how to further diagnose this.

Steps taken so far:
 - "quiet splash" removed
 - nvidia drivers installed from nvidia's website (310)
 - nouveau driver blacklisted

ALSO ALt-SysReq REISUB does not respond ever. The only way to recover from these failure boots is to power cycle.

Kernel does not successfully boot for me to run required command. Recovery mode also fails to boot.

Managed to boot into system by:
  - Power cycling multiple times, booting regularly would bring to a similar situation as https://bugs.launchpad.net/ubuntu/+source/linux/+bug/984127
 - It would complain that fschk needs to be done, however, no button press or command would respons
 - I kept power cycling, let it sit for a bit, power cycled, tried recovery mode a few times, that would just freeze.
 - One time (this time) recovery mode worked, from there I did resume normal boot.

Do you happen to know if this bug existed on previous versions of Ubuntu, or have you only used Precise(12.04)?

Would it be possible for you to test the latest upstream kernel? Refer to https://wiki.ubuntu.com/KernelMainlineBuilds . Please test the latest v3.7 kernel[0] (Not a kernel in the daily directory) and install both the linux-image and linux-image-extra .deb packages.

If this bug is fixed in the mainline kernel, please add the following tag 'kernel-fixed-upstream'.

If the mainline kernel does not fix this bug, please add the tag: 'kernel-bug-exists-upstream'.

If you are unable to test the mainline kernel, for example it will not boot, please add the tag: 'kernel-unable-to-test-upstream'.
Once testing of the upstream kernel is complete, please mark this bug as "Confirmed".

Joseph, this bug has existed since the unity change. I have had trouble with 10.10, 11.04, 11.10. The last stable kernel/release to run on my system successfully was 10.04.

I have the same problem on Ubuntu 12.10.

Ubuntu starts only if Nvidia card is turned of in BIOS (Intel card is selected).

@Constaintin,

In the bug description, you mention that you left 12.04 and returned. Did you perform a fresh install each time?

Also, does the bug go away if you disable the Nvidia card like mentioned in comment #27?

Joseph, yes every time was a indeed a fresh install. I never attempted to disable the NVidia card in the BIOS. I did however, attempt the opposite, enabling optimus and using the nouveau drivers. People suggested they had luck with that, however I remember not succeeding.

Currently, I am working with 12.04. My current solution has to been to stay booted once I boot successfully... I've noticed higher success with the following setup:

  1) nomodeset - once when Nvidia's drivers are unused (during a kernel upgrade for example). It's important that I remove this once the drivers are compiled and installed for the new kernel otherwise I do not boot.

  2) Empty boot lines for the kernel, in other words removing "quiet splash".

Hi Joseph,
  Updated my kernel headers today, did a reboot. Added "nomodeset" at grub using 'e'. Booted successfully and recompiled NVidia drivers. Did a 'sudo service lightdm start' successfully and finally rebooted once again.

  At reboot, I got stuck at a purple screen (even though "quiet" and "splash" are removed). I had to power cycle, let it sit for around a minute. At boot, successfully saw status messages and booted. (note neither of these were with nomodeset).

  Lastly, I would like to make note that I am docked and attached to two external monitors. Thanks.

Good Evening and Happy New Year,

   Attempted a reboot again after software install. This time no kernel update, so no "nomodeset" at any time.

   First, direct "Restart" attempt I got stuck at a purple screen. I had to powercycle.

   Second attempt, a power on instantly after the power down, I got status messages but then I got stuck. I'm unsure of which status message I got stuck at as I'm confident that I'm simply seeing the "top" of the console buffer. I had to power cycle.

   Third attempt, a power up after waiting around 30 seconds. Successfully booted, I did notice a status message along the lines of
   "Initializing back up display device [failed]" or something similar.

   Is there any new information on this issue? Is there any way I can help you debug this issue? Thank you.

Good afternoon. After a kernel upgrade yesterday I am unable to boot into 12.04.1 again. I managed to boot in initially using "nomodeset" and reinstalled my Nvidia drivers (310.## I believe). However, now I simply get stuck after a "loading swap" message.

Is there any new information on this issue? Has it been acknowledged? Could anyone provide some support or guidance. I would be happy to help debug but I have no idea how to approach this.

Hi Joseph,

  I followed the instructions as you suggested and installed the 3.8-rc5 kernel. I did not see improvement, thus I added the kernel-bug-exists-upstream tag (I may needed to have added the kernel-unable-to-test-upstream.)

  Here's what happened:
     - nomodeset was not a guaranteed a boot like usual on a kernel boot with no compiled nvidia drivers
     - I saw similar situations as before: blank purple screen, getting stuck at the same status message ( something about loading swap )
     - I did manage to boot once (no nvidia drivers) but I was unable to use my mouse or keyboard... I assume this is because there were no drivers installed with the kernel ... was I supposed to do that?

I finally managed to boot back into my newest installed "3.2.0-36-generic #57-Ubuntu" but I had to do it through the recovery menu. The previous kernel (35) didn't have this problem quite so badly... Perhaps it's the 310.19 Nvidia drivers?

I'm not sure if it's related, but I'm also seeing that on a T520, I'm *consistently* unable to boot with discrete graphics enabled. Either Integrated or Optimus boots fine.

Constantin Savtchenko, as per http://download.lenovo.com/express/ddfm.html an update is available for your BIOS (1.43). If you update to this, does it change anything?

If not, could you please both specify what happened, and provide the output of the following terminal command:
sudo dmidecode -s bios-version && sudo dmidecode -s bios-release-date
