USB Pen is not working

I have a Lexar 32GB usb pen that doesn't work.
When I try to format it with Disk Utility I get:

GParted can't see the device.
Is there a way to fix this?

If you plug it in and run:

sudo fdisk -l; lsb_release -a; uname -a; mount

What file system is the device?
When you last unplugged it, did you use the safetly remove device feature in the OS?

Not sure, most of the times I do, but sometimes I unplug the pen after the computer shuts down.
By the way, is there a option to directly asssociate the questions to other aspects of the operating system?

About the file system, I'm not sure too. It was created a few years ago and I don't remember now. Is there a way to know it now?
Perhapps it's irrelevant now, most certainly I'll have to format the pen if i manage to get the system to recognize it.
