changing default location of home folder in ubuntu 12.04

I recently installed ubuntu 12.04 alongside windows 7.

I had all my media stuff in a separate partition and so i want to change the default location of home folder menus ( like documents, downloads, etc.) to that partition.

I used ubuntu tweak for changing it.

Though i was able to change the default location but after rebooting changes were undone and nautilus got its default location back!

How can i fix it permanently?

If it's just recently installed it might be easier to reinstall and create a separate /home.
