The problem is trying to sign in or open Ubuntu my program pass word ?

Never able to use the same pass word any time I need to open and or upgrade Ubuntu ?
How can I correct this problem , I wanted to upgrade to Ubuntu 12.04- But when asked for
 password. I type in the last one that I had entered in my book,but is the wrong one
so that as far as I can go. Any help at all will aid to or maybe correct this problem

Try:

sudo apt-get update; sudo apt-get upgrade; sudo apt-get dist-upgrade

The updates may help fix the issue....
