It is possible that you are using an icon theme that is not compatible with OOo.
Try switching back to the default theme and see if it works again.

Thank you to reply, but I'm newbee with OOo. Can you describe what is the Icon Theme? I don't change any default settings of OOo. This problem occurs while I upgrade my Ubuntu from Edgy to Feisty.

I'm talking about the desktop icon theme: Menu->System->Preferences->Theme select Theme on the left, Customize on the right, Icons tab.
Some icon themes are not compatible with OOo (actually, I believe they just need a matching theme to be installed for OOo).
You can also check in OOo if by any chance the toolbar setting was changed. For instance in Writer Tools->Customise, Toolbar tab then Toolbar on the right Icons only (or Icons and text).
Let us know if none of the two solve your problem and will take it from there.

I have this same problem. I just installed kubuntu feisty 23 July 07 and did the
upgrade today. During the upgrade I received device 169 errors. As near
as I can tell the icon theme is the default CVS. I am using my home
directory with settings from Kanotix and previous linuxes. I don't know
how to reset the themes in open office--if that is the problem.

Same problem here using Ubuntu Feisty. I was using the SphereCrystal theme and tried changing the icons to Human which made the icons in OO return. Human theme icons don't look good with SphereCrystal so I'll have to change my default theme.

For Kraftway and any other newbie: To modify themes go to "System" then to "Preferences" then to "Theme" then "Customize".

Hope this helps,

John
