How to create a fill in the blanks survey document to send out via email

This is not a problem just a request to know how to do it.

Is there a way in OpenOffice writer to create a fill in the blanks document that can be emailed to others to fill in the blanks and send that document back to the survey taker.

I use google docs www.docs.google.com. Go under file-->form. It will let you create surveys send them out then see the results in a spread sheet.
