2nd display (laptop to monitor) is pixelated/fuzzy on boot until you turn off and back on (see video)

I have a minimal Ubuntu 18.04 installation on a ThinkPad X240 with a monitor plugged in via Mini DP (laptop) to HDMI (monitor) cable.

When turning the laptop on 1 out of 2 times it looks like in the video below and keeps going black and then coming back:

If I turn the monitor off and back on the display is then fine.

I have run 16.04 for a year on the same hardware without any issues - this only occurs since switching to 18.04.

What is the output of:

sudo lshw -C display; lsb_release -a; uname -a;xrandr

The last message was after I switch off the monitor and back on (so the screen goes back to normal).

Here are the results if I do this when the screen does it's thing (when I login the monitor turns off completely - again if I turn the monitor off and back on it's fine):
