Installation crashes when attempting to install on UEFI and mdadm enabled system on existing GPT partitions

Could use a guide to help ensure all partitions are assigned for uefi to boot properly. From what I've seen online I have created a proper BIOS boot partition, but failed to create an EFI partition.

bios_grub partitions are for bios machines to boot gpt disks, not uefi. uefi machines need a fat32 boot partition mounted in /boot/efi.
