Wifi drops out , can't connect lenovo g50 45, Ubuntu 12.0.4

I tried several methods but still no luck.
Wifi keeps asking password every 5 minutes and there is no access to internet through wifi.

Ubuntu 12.0.4 is installed on Lenovo g50-45 laptop.

This is the problem

Just selected 'ignore' option in Edit connections->Wireless->IPv6.
