Iball 3.5G USB modem not working in Ubuntu 12.04

I have recently purchased an Iball make 3.5G USB modem but this modem is not working in Ubuntu 12.04 version. It works well with Windows 7 (I have dual boot system with Wubi installation). The response from lsusb and uname -a command is as below:

sudo lsusb output is as below (when the device is connected):

Please help with step by step procedure of making the USB modem work on Ubuntu.

05c6:0016 is your IS, just change it for the one in the guide
