Can you send the same application again? Do you mean a fresh application?
If it is a .odt file, can't you edit it and make another pdf and send again?

If they want you to fill in the document, then why did they send it in .PDF format without any explanation on their part?

It does not make any sense.

Can you ask the sender to resend the document in Word .doc format or ODF (Libreoffice ODF) format?

It would be even better if the sender would simply send you a link to a web based survey or questionnaire.

If they use a webbased questionnaire system, it would save a lot of diskspace on the sender's and receiver's mail inbox.

I suspect that the root cause here is not a technical problem, but more a problem with the whole process flow and the whole way of working between sender and receiver.....

Maybe they expect you to print it out, fill in the form, and mail it back? Mark is probably correct, and talking to the sender to figure out the solution may be best. However, I think xjournal might actually allow you to fill in the form, and either save the info you supplied in a small file which can be reapplied to the pdf to "fill it out" or I think even regenerate the pdf with the info included (creating much bigger file).

You need to save the PDF Form and then open it with the Document Viewer (Evince).

The built-in PDf viewer in Firefox is for viewing only, not for completing PDF Forms.
