Evolution Address Book Import Data Mapping

I need to import Address Book from MS Outlook 2003 into Evolution 2.12.1.

Have tried standard Outlook 2003 export as CSV - Data mapping in Evolution results in many fields missing, or inserted into incorrect Evolution fields

I also run Thunderbird so attempted an CSV export of Address Book, the import CSV into Evolution, same result as with Outlook 2003 CSV export (although with this, at leas the name fields were correct).

To overcome the problem, I need the import field mapping from Evolution 2.12.1, and I can then remap the Outlook data to suit Evolution.

Any help would be appreciated.
