I can't print my driving record.

I entered the information needed to request my driving record. Then when it was time to print the report, a message popped up saying that "The document 'form-1.pdf' is locked and requires a password before it can be opened." Earlier, I recall seeing on the printing tips that the password was on my drivers license. HELP! What do I do?
