compiz fails unexpectedly

I get sporadic reports that compiz has failed in ubuntu 12.04

Can you give the output of:

sudo lshw -C display; lsb_release -a; uname -a; apt-cache policy compiz

Output as requested. I struggled a bit to get a terminal window up, my dash home seems to have lost all the applications that it normally offers. Eventually found the keyboard short cut.
