Couldn't upgrade to 12.04

I tried upgrading using the upgrade manager but nothing has happened, then i used this command to upgrade

"sudo do-release-upgrade -d"

And its also didn't work, this is the output that i got

Hi.
There is accured bug. Please report it that developers can fix it. You can still upgrade your ubuntu version by upgrading it with Upgrade manager.

Hello Miha
I have no clue how to report the bug. Also i can't upgrade to the newer version using my Upgrade manager, i get a partial upgrade screen :s

Hi ,

can you please attach the 2 files ?

You can still upgrade from an Ubuntu Live CD or Usb.

Download the iso image , burn it to a CD or Usb , boot from there and choose the option "Upgrade Ubuntu <release nubmer> with Ubuntu <release number> " from Ubuntu Installer.
This option will (try) upgrade your current Ubuntu Installation , without delete your personal files.
