Your ALSA mixer settings show that the Master volume channel is muted in gnome-alsamixer:

Please unmute it.

# Simple mixer control 'Master',0
# Capabilities: pvolume pvolume-joined pswitch pswitch-joined
# Playback channels: Mono
# Limits: Playback 0 - 64
# Mono: Playback 50 [78%] [-14.00dB] [off]

Your output from step 4 shows the issue your pc is having:

/dev/snd/controlC0: Slmodemd 1161 F.... slmodemd

Your Si3054 Modem is hogging the /dev/snd devices, which is preventing your soundcard from working correctly.

Please try this solution:

https://answers.launchpad.net/ubuntu/+source/alsa-driver/+question/87927

which mentions this bug:

https://bugs.launchpad.net/ubuntu/+source/pulseaudio/+bug/394500/comments/40

So you need to disable the Si3054 Modem (56k modem)
