Try deleting pulse configs from your home directory in the terminal:
rm -rf ~/.pulse

Then restart pulse
killall pulseaudio

Then check alsamixer
alsamixer

Everything should be green (unmuted). If not arrow and press m to unmute.
Press Esc to quit.

I recommend doing a clean install of Ubuntu 9.04 (Jaunty) and then installing all the updates for Ubuntu 9.04

This also works in Ubuntu 10.04 Alpha 3.
