As already written in https://answers.launchpad.net/ubuntu/+source/alsa-driver/+question/701260 :
Your output shows a crash with qasmixer.

I do not know anything about that program, so I cannot help further, but in my opinion there is a high probability that this is the root cause.
Reinstalling pulseaudio will not help if there is a problem with qasmixer.

qasmixer is not the issue, it was purged from the system & the problem continues.
The instructions you suggested
https://help.ubuntu.com/community/SoundTroubleshootingProcedure
caused the issue

Why does it still show in your outout?

[18501.182003] qasmixer[26747]: segfault at f8 ip 00007f4aac4335a5 sp 00007ffe170cd970 error 4 in libasound_module_ctl_arcam_av.so[7f4aac432000 3000]

Have you rebooted between uninstalling the qasmixer program and running the alsa info script?
