speakers and/or headphones only work on restart ubuntu 16.04

Running Ubuntu 16.04. When I boot up, I can play sound through the internal speakers. If i plug headphones in, the system detects the headphones, and I can select them in the sound preferences, but no sound comes out from the headphones or the internal speakers, even if I remove the headphones from the jack.

If I boot up with the headphones plugged in, the sound works through the headphones. But if I select internal speakers (on system preferences) or remove the headphones, the sound stops working again.

In both cases, the only way to get the sound back is to reboot. I have tried most of the steps listed here: https://help.ubuntu.com/community/SoundTroubleshootingProcedure.
