Your Terminal output shows this:

Simple mixer control 'Headphone',0
  Capabilities: pvolume pswitch
Playback channels: Front Left - Front Right
  Limits: Playback 0 - 87
  Mono:
  Front Left: Playback 0 [0%] [-65.25dB] [off]
  Front Right: Playback 0 [0%] [-65.25dB] [off]

Use alsamixer in a Terminal to unmute and increase volume to 100% on ALL mixer channels, including the muted Headphone channel....

Trying to use different model options at the same time - as you did below - will not work. You can only test ONE model option at a time between each reboot.

snd_hda_intel: model=dell-m6-amic
snd_usb_audio: index=-2
snd_hda_intel: model=laptop

Can you help with this problem?
