sudo add-apt-repository ppa:ubuntu-audio-dev/ppa; sudo apt-get update;sudo apt-get dist-upgrade; sudo apt-get --reinstall install pavucontrol linux-sound-base alsa-base alsa-utils; killall pulseaudio; rm -r ~/.pulse*

Wait 10 seconds and reboot.

Please use alsamixer to UNMUTE and INCREASE volume to 100% for the following mixer channels:

Simple mixer control 'Master',0
Simple mixer control 'PCM Loopback',0

I had already done the updates actionparsnip suggested (and more) from advice in other posts. They must have added some new options to alsamixer because when I make the changes Mark suggested (including a Speaker option), everything now works!
