Shut down causes restart instead of staying off

When I shut down my computer, It shuts down momentarily, then turn right back on. Once powered on, it no longer has power to mouse touchpad or keyboard. The only thing I can do is hold down power until it powers off. Also, any time it goes into sleep mode, it shuts off screen, mouse and keyboard. Without mouse or keys, you can't wake it from sleep, therefore you can only do a hard restart. It also doesn't turn on controls around half the time it starts up. I have had to turn off all sleep functions and the only way to safely shut it off is to do shutdown, and once it restarts, kill it with the power button during the bios boot.

Does the system have a make and model?
have you tested your RAM health?
What is the output of:

lsb_release -a; uname -a
