send report internal error 16.04

Good afternoon,

After launching 16.04 LTS I got message that internal error occurred. Ubuntu is not working as should, when clicking sidebar left, screen is flickering, falling out.
How can I send you a report what is going wrong. I launched Ubuntu 16.04LTS from USB stick - try out.
