Error while login & updating 16.04.2
While booting it show's " The system is runing in low graphics mode" and sometime "initramfs" don't know why it show the error repeatedly after resolving the problem. Last time had received a update of lunix generic from terminal with upgrade & dist-upgrade. After reboot boot it showed error and unable to boot again with low graphics mode & sometime "initramfs". Both tried to resolve but error comes up with update and i am using live cd to resolve problem.

Tried to resolve by online help using command :
sudo rm -vf /var/lib/
sudo apt-get update

Why are you using sudo if you are already root? Do you log in to the desktop as root?

Andrew i was able to login and i am using my desktop. while login it showed some error problem report and was send. run the command apt-update, upgrade, dist-upgrade with no error. now it working but don't know what will happen on next boot. let me wait and see. Thanks for your assistance and still unable to understand why this frequently happen with my system.

My conclusion at that time was that you have a hard disk problem.
My assumption is that you are still suffering from it.
What is the output of the command
dmesg | grep ' failed command: READ'

Andrew still the same issue when i rebooted today. Unable to boot showing error and after that "initramf" Done hard disk check with command :
sudo fsck /dev/sda5
sudo fsck -nf /dev/sda5
And after it used commands to resolve "initramf"
Now again i am using live cd and same issue with update.

You have to understand that running apt-get and dpkg commands from inside a live system does not make sense at all (unless you have chroot-ed I into your hard disk installation).

So its my Hard disk issue. Already replaced many time with new hard disk and not happy sea-gate hard disk. Its a well know brand but i had suffered with failure many times before. Don't know which hard drive will have a better quality with good life.
Manfred you suggest me to change hard disk. already i have lot of data in drive.
