Ubuntu 16.04 LTS, no sound, builtin speakers, Intel Corporation 5 Series/3400 Series Chipset High Definition Audio (rev 05)

I've went through all relevant steps from the Ubuntu sound troubleshooting guide (https://help.ubuntu.com/community/SoundTroubleshootingProcedure). I have no sound through builtin speakers, but I get sound from my headphone jack. Here is the output from step 3 and step 4 in the sound troubleshooting guide:

In summary, I've made sure my sound is NOT muted, I've checked pavucontrol to make sure my system sounds is at 100%, and checked alsamixer to make sure almost all audio channels are maxed. I've also tried reinstalling alsa. I've checked some other posts online, particularly here on launchpad, and nothing has helped so far. Any suggestions?

If you run:

killall pulseaudio

Then wait 10 seconds, does sound work OK?

Nope, `killall pulseaudio` doesn't work.

However, I've had cases where when I put my machine to sleep/suspend, then wake it up, sound works for awhile. I've only been able to reproduce this case twice. Seems intermittent.
