Wifi Can't Setup New Connection

I am using Xubuntu 16.04. I have set a wi-fi connectivity and its working fine. My system details are :

Problem comes when I want to set up a new Wi-fi connectivity for access points showing in nm-applet list. The set up dialogue box is not opening and the applet keeps on rotating and even shown as connected without asking for password etc.

I did reinstall my BCM4313 driver with following :

and then restarted wifi with following
