leadtek DTV 2000 plus card installation

Could someone please point me in the correct direction to install the above tv tune card. I am running UBUNTU 12.04.4 lts. I have followed steps over the net without any luck.
Ifnot please recommend a TV tuner card that will work?

The make and model does not tell us enough about the device. We need the chip on the card to be able to advise.

What is the output of :
