efi disk read error

Getting 3 Errors on Bootup on system running Ubuntu Server 12.04.2 LTS
 efi disk read error
efi disk read error
efi disk read error

Then the System boots fine no errors at all system seems to work fine. but still get the errors every bootup.
This System is Dell Precision T7600 System.
Running Raid 5 Via Perc h710
Set to UEFI Boot.
with seperate /boot Partition.

Installation goes Well and system still works fine.
just get these errors on Bootup every Time.
Only Operating System installed in Ubuntu Server no not issue with Windows Fast Startup as it not installed.
Cannot Disable Secure Boot as not even an Option in the Bios.

drive Container was 4 TB +

one EFI Boot Partition.
/ Partition
/Home partition
Swap Partition

so has 4 partitions in total.

Maybe each partition is tried at boot. Not 100% sure but maybe other users can advise.

Here's an idea::have you tried booting into recovery mode and checking all fs for errors? No efi or uefi do not try each partition, but also trying the fix broken packages option in the recovery menu may help.

FS don't have any Errors at all. Full fsck on all Partitions no errors.
No Broken Packages Fresh install of the OS Direct Download from Repos.
and have got 2 Computers in Different Locations exactly the same Errors.

Fresh install and on First boot errors is present.

Install 12.10 and its not present
only issue is this is SERVER install and need LTS .

I Suspect it is coming from Grub 1.99 as it is pre Kernel Load.

An easy way to tell if it's grub:

Press shift on startup. If the issue is with grub, you'll either see these messages before the grub screen, or immediately after selecting any boot option. Is there a reason you're using a seperate boot partition? Those can sometimes cause problems with grub.

^ Sorry, I didn't realize you were using efi! The seperate boot partition won't be the problem and should be left alone.

Have Confirmed it is not becuase of the Number of Partitions as i Reformated the system and reinstalled a Brand New installation on only 3 partitions
250MB EFI Boot.
900GB / Partition
90GB Swap

and it still comes up with the 3 Errors on Bootup,

This is Before the Grub Starts as errors appear before the Grub Splash Screen Via SHIFT on boot,

Should this be Transfered to a Bug Report and if so what other info is required.

Why 90Gb for swap!? Never seen anything like that in my life. Do you run a lot of SQL or something?

Does it really matter how much the swap space is it was just a test Server to show it wasn't the number of partition it was just the left over space from creating / as 900GB

This is not the reason for the errors before the grub splash screen.

If it's before the splash it's either firmware or more likely grub, but the swap won't affect it at all. I'm not certain that I have the knowledge to help any further. I'll look it up.

Using Ubuntu 12.0.4.4 LTS same issue present.

Did find that booting to a Live USB Key and using PPA ppa:yannubuntu/boot-repair

the Live i was using was 13.10 Live and running the boot repair tool just removed the tick from Secure boot did fix the issue no more error message

so as the hardware doesn;t have Secure Boot as option it is looking like the Grub loader by default is enabling the secure boot by default and disabling it fixed the issue.

the only thing i need to confirm if doing the same thing with using a 12.04LTS Live boot if this will fix the issue.
It is looking more like an issue with the secure boot options in the 1.99 Grub
