Thanks for the output,
this issue should be fixed in the latest kernel already (from 4.4.0-58.79), I noticed that you're using kernel 4.4.14-040414-generic. Which doesn't looks like the one shipped with Xenial, I will suggest you to install the latest stock kernel and try again.
