HP Z640 Workstation - USB does not work in ubuntu 16.04 with kernel 4.4.0-22.38 and above

On HP Z640 Workstations running ubuntu 16.04 with any official ubuntu kernel newer than 4.4.0-21-generic USB devices do not work. They appear to be listed correctly via lsusb and connects and disconnects are logged but keyboards, mice, storage devices, ethernet adapters do not work at all when they are connected directly via any USB ports on the machines or through USB hubs.

All ubuntu packages are up to date. The BIOS version is the latest (2.29).

The ubuntu 4.4.0-21.37 kernel works correctly as do older kernels from ubuntu 16.04. Vanilla kernel.org kernels that I've tried such as 4.4.30 and 4.4.38 work correctly.

Ubuntu kernels greater than 4.4.0-21.37, from 4.4.0-22.38 all the way to 4.4.0-59.80 and 4.8.0-34.36 are affected by this issue.

I'm happy to provide further diagnostics but this issue should be easy to reproduce with the appropriate class of machine.

Any suggestions on debugging this further are much appreciated since until this is fixed we are going to be forced to use old ubuntu kernels or vanilla kernels.

Hello,
this appears to be a regression, as you mentioned that it works for kernel older than 4.4.0-21.37

Let's focus on official Ubuntu kernels first.
I have convert this into a bug. But we need your help to collect hardware information, please boot to the non-working Ubuntu kernel, and run "apport-collect 1661170" in a terminal.
