form factor

Hi,

With Madgragh5, I want to add a form factor to anomalous quartic gauge couplings so that they violate the unitary at high energy. However, I didn't quite understand how to include the information in the add to form factor part in the anomalous vertices. Here, I would appreciate if you could give an example of adding a form factor to any coupling in the presence of anomalous couplings for the process p p > w+ z a.
