Hello,

Probably some server problem, recently there had been problems with ubuntu servers and launchpad servers, so try it again. If the problem persist than you should look through the form, probably you have done something wrong.
If you still have problems than there could be a lot of answers for your problem. Ex your browser settings does not allow cookies.

But anyway write in details about your problem to be clear for us
