Please rename moniker and python-monikerclient

The Moniker project has been renamed to Designate, can we please have the two projects renamed as follows:

moniker to designate
python-monikerclient to python-designateclient

Done. They're now https://launchpad.net/designate and https://launchpad.net/python-designateclient, and I've added redirects from the old names.
