How do I create a fillable PDF form?

I'm trying to create a survey to email out to our clients in pdf form where they can type in short answers and email it back

Hi Greg,

Inkscape is a drawing program - as far as I know, it does not have the functionality to create pdf forms.
I think you should better look at an Office program for this kind of purpose, for example LibreOffice or OpenOffice. Those provide a (quite) simple to use interface for this kind of task, I have successfully used it myself.
