How to place flag background in cut out clip art shape

Hello,
 I know how to create a flag background within text, but it doesnt seem to work with a cut-out silhouette/drawing. I want to do the same thing, not with text but with a clip art drawing. Example- Instead of a black silhouette of a person, I want to replace the black, with a flag image, in the shape of the person.
