The 'Camera and Lens Data' window is not appearing when I load an image to convert from cylindrical to planar format.

The 'Camera and Lens Data' window is not appearing when I load an image. This is necessary to convert from cylindrical to planar format. I have un-installed and re-installed Hugin but with no success.

Hugin has remembered your last inputs (for crop factor and projection) for this lens and is automatically using it now.
You can also change this settings on the photos tab in the panorama editor (without this dialog).
