header is cut off when printing

Whenever I print a page, the header, which contains the title of the page and url, is cut off. Also whenever I print a page, regardless of the number of pages, the printer always ejects a blank page at the end of the job. I would like to change the printer from cutting of the header and spitting a blank page at the end of each print job.

I wish I could attach a file of the log that was created but since there's no attach feature, here's the log:
