printing extra printer description along with print job

When I print a job the job prints then another job prints on a seperate sheet that includes:
Media Limits:, Driver:, Driver Version:, Description:, Driver Version:, Make and Model:, Printer:, Created at:, Printed at:.
How do I get it to print just the job?

Hello Steve,

Normally extra page is printed if we set banner printing in Cups. Have you enabled banner page printing? You can check the same using below steps.

1) Open http://localhost:631 in the browser.
2) Go to printers and then select the printer for which extra page is getting printed.
3) Set Default options -> Banners(Tab)
4) Let us know the values set in "Starting Banner" and "Ending banner".

Hi Steve,

We need some more logs to analyse this problem.
Can you run command "sudo hp-logcapture" in terminal and attach the file hplip_troubleshoot_logs.tar.gz generated by this command in the link https://answers.launchpad.net/hplip/+question/253304/+makebug
