Unwanted report page on printer

Printer
 Officejet Pro 8100. Prints a summary page with each print job, detailing Media Limits, Job ID, Driver, Description, Make and Model, Printer Created at, Printed at. This wastes paper, particularly if I am printing photographs, and it takes some time to print the report. OS is Xubuntu 12.04, and the report is printed by any app that can print, documents or photos.
I have run hp-check -t and the result is:

Hello Ian,

Unfortunately I am not able to reproduce this issue here. From the logs I see that both the print queues are in a disabled state. Can you try below steps.

=> Download and install the latest HPLIP release from
http://hplipopensource.com/hplip-web/install/install/index.html (automatic installer)

=> Reconfigure print queue using below commands.
       => system-config-printer (remove all print queues)

       => hp-setup (It should not be run in a root mode. make sure you select ppd name having "hpcups" substring)
=>Try printing now.

If above steps do not work then install cups-1.5.3 from cups source.

If upgrading/re-installing of cups also does not work then please upgrade/reflash printer firmware from hp.com.
