HomeBank - csv file format import

I am new to Homebank. I'm trying to import a simple database out of a Windows app into (K)Ubuntu Homebank using a generic .csv file format. The file loads fine into OO Calc. I must be blind but I can not find the sample .csv file on Max's site (https://launchpad.net/~mdoyen)? or what a proper file suffix is. When I get to the import panel in Homebank, the csv option is greyed out and will not recognize files with a xxxxxx.csv file suffix.

Any help in this area much appreciated.
