Menu Bar not integrated after disabling and enabling again

after disabling the Global Menu Bar integration extension and subsequently enabling it again (after restarting Firefox each time), the menu bar was NOT integrated again. I don't know how to get it working again.
[I also noticed a blue demarcation on the left side of the browser window when the extension was enabled. it is gone now. what is that for??]
